Grace: Meet Grace...Meet Needs
Grace was born in 1988, and was diagnosed with autism at the age of five. Grace’s early years proved to be a mix of fascinating and challenging experiences for her and her family. They have explored many avenues through the healthcare system, some more successful than others. We are fortunate to hear about their experiences and the wisdom gained from this journey. Grace showed an interest in art from an early age. Over the years, her love for artistic expression has evolved from a fun pastime to a blossoming career. As an individual affected by autism, Grace experiences heightened sensory perceptions. She is believed to have Synesthesia, the intermingling of senses, where music has colour, and colour has smell. This sensory acuity is also visible in her art, as she combines patterns and textures with strong use of colour in ways that reflect both the chaos and beauty of her experiences living with autism. Momentum Choir Website
Momentum choir was established in the Niagara Region in 2007, through the vision of Executive and Artistic Director Mendelt D. Hoekstra, with the assistance of Bethesda Community Services and a grant from the Ontario Trillium Foundation. It is a registered charitable organization and operates under the guidance of a Board of Directors. Since its inception, Momentum has grown to include over 45 talented singers and musicians who are passionate about sharing their joy in music with the world. It has become a sought-after choir that carefully chooses the events and venues at which it will perform. Momentum rehearses weekly in St. Catharines, and all singers are invited to a placement audition.

SHARE Transition Plan: talking about transitions with young people and families.
Transition to adulthood can be daunting for people with developmental disabilities. The Developmental Disability Primary Care Program (DDPCP) developed SHARE Transition Plan Tools to guide for health care providers to communicate with patients and families about their transition from pediatric services. SHARE spells out 5 key areas for planning transition: Supports for Communication & Capability; Healthcare Transition & Transfer; Activities & Engagement; Relationships & Well-being; Exploring Services & Supports. Transition planning involves setting goals with youth and families in these areas over time. “SHARE” is also the essence of the collaborative process. 5 unique families SHARE their experiences with these 5 realms of shared transition planning. Communicate CARE when you INTERVIEW
Clinical encounters can provide a portal into understanding patient needs and person-centred care. Communication in interviewing is vital: communicating caring attitudes and actions in tandem. Some patients and populations require extra consideration in clinical settings. Many individuals with developmental disabilities present with cognitive, communication, motor or sensory differences that necessitate adaptations as we prepare and engage them in interviews. The following guidelines provide a summary of steps to “Communicate CARE when you INTERVIEW” a patient with a developmental disability. The principles and practices are broadly applicable and stand to benefit patients as well as clinicians in initial clinical encounters.
